How to Order the User’s Guide

The User's Guide for Duke Health Measures can be ordered on-line or by mail.

1. Both the electronic version and the printed version can be ordered on-line.
2. Click on Order User's Guide On-line to go to a secure site where you can place your order and enter your VISA or MasterCard payment information.

To order by mail:

1. Only the printed version can be ordered by mail.
2. Download the Order Form by clicking User's Guide Order Form.
3. Complete and return the Order Form by mail. Indicate if you want to pay by check, money order, VISA, or MasterCard.
